Pulse check for mobile checkout

Would love to be able to do a quick survey/pulse check for mobile check out. Something similar to "how was your stay?" with a 1-10 rating. Then, for example, if the guest gives us a 6 or below, it advises the guest to either call a phone number, stop by the front desk, or email the guest so we can solve the issues prior to the guest leaving. It would also be nice if we can be aware of the guests who gave us low scores so we can reach out to them if they don't reach out to us. The only danger with mobile check out is that we miss the opportunity to ask a guest how their stay was and to get their direct feedback.
